Interested individuals may apply in one of two ways:

by completing and submitting a “hard-copy” application
by downloading an on-line application.

To view and print the on-line application you will need to install Adobe Reader.

To obtain a hard-copy application, you may:

Pick up an application at CDTA, 110 Watervliet Avenue. Application forms are available during regular business hours from 9 a.m. to 5 p.m., Monday through Friday.
Call (518) 482-2022 to obtain an application by mail

Hard-copy applications are available in accessible format upon request.

NOTES:
We may not be able to accommodate you if your wheelchair/scooter is wider than 31”, longer than 48”, when measured 2” off the ground, or if your total weight with your wheelchair is more than 600 pounds.
